 Our experience

We support people with complex needs due to learning disabilities or autism. We can help young adults who are moving from children’s services to adult services to develop their independence and to have choice and control over the support they need to achieve their hopes and dreams.

At the core of what we do is the person we support so we plan services around each individual person and their needs.

How can we support young people in transition between children’s and adult services?

  • We can find accommodation that meets the needs of each individual.
  • We can support young people to develop new skills in order to be more independent.
  • We can support young people to engage with their local community and enjoy all it has to offer.
  • We encourage people to enjoy family life, make friends and lead a full life.

What does the service deliver?

  • We ensure that the people we support are safe and happy.
  • We provide person centred support, working in partnership with individuals, their families and healthcare professionals.
  • We work with individuals and their families to select their own support staff.
  • We provide skilled and experienced support teams who undertake specialist training in relation to the person they are supporting.

Who is the service for?

We can help any individual approaching the age of 18 who has been assessed as requiring care and who would like us to start planning their future support.

What is next?

  • We will meet with you to find out your needs and wishes.
  • We will work together with you to develop a support plan and a person centred job description which will enable us to find the right support staff for you.
  • We will manage your service for as long as you would like us to.
